Both Grokh and Terminus each have an ability to not die when they would get killed. For Terminus it is his ultimate, for Grokh it is his talent Wraith. For some reason it is shown both team and enemy that they will not die on a lethal hit:
This is really bad for Terminus, as it will give away to the enemy, that he has his ultimate ready. Even before he dies. He is the only champion in the roster who cannot surprise the enemy with his ultimate because of this bug.
