A fix was implemented for the following bug but it still occurs prominently. I'm not sure if PC players are also experiencing it but it has happened to me many times even after the patch where a fix...

Also happens on PS4, I may be wrong but it usually happens if you die after casting an Astral Mark on a team mate.
